Blue Cross Blue Shield is a health insurance company, their agent need to fill in a lot of Excel forms. They want to turn those forms into online forms, so their agent can use it wherever they are.

My task is to design a long form user experience, user flow, and user interface design. The hardest part is to type in all text in 4 forms by using moqups.com.

This is the original length of the forms. You see, it's super long!
Here are some screenshot of part of the forms. On the left side, there is a floating menu, which can help users to navigate. Users can open a case by search or open from the recent cases. Users can also do save or save as when they complete forms, the saved form will turn into a PDF file in the end.

Overall I like this design project, and I really like the color pattern I chose for those forms, even though the typing part is tedious.
